Long Term Support Services (LTSS)
The Changing Lives Center provides a range of services for adults who need additional help in order to continue safely living in their own home. Some of the services that the program provides include bathing, grooming, medication reminders, meal preparation, housekeeping, companionship, and more.
Get Assessed
A nurse will visit you at home to determine if you’re eligible for the CDS program and how many weekly hours of care you can get.
Choose your caregiver
You choose a caregiver: your daughter, son, relative, or friend — anyone except your spouse. We come to your home to enroll you in the program.
You get care — They get paid
You get ongoing care from a person who loves you . They get compensated for their time, so they can pay their bills too!
